Medicaid Waiver Program

The Medicaid Waiver Program is funded through the Michigan Department of Community Health. The program is designed to provide home and community-based services through Medicaid funds to frail, elderly or physically disabled adults who would otherwise require nursing home care. Services are provided at no charge to participants meeting medical and financial guidelines.

Program are available through HHS, Health Options in: Allegan, Ionia, Kent, Lake, Mason, Mecosta, Montcalm, Muskegon, Newaygo, Oceana, Osceola, and Ottawa Counties.

Description of Services:

• Experienced Registered Nurse and certified Social Work Case manager evaluate participants
• Assistance in understanding long term care choices
• Provides comprehensive assessment, care planning, monitoring and reassessment services
• Follow-up and ongoing reassessment for all participants
