Kent County Senior Millage

Care Management and Home Support Programs

The Care Management and Home Support programs, funded through the Kent County Senior Millage and administered by the Area Agency on Aging of Western Michigan, are designed to assist frail or disabled Kent County residents age 60 and over maintain independence in their homes.

Description of Services

• Comprehensive assessment by experienced Registered Nurse and certified Social Work Case Manager
• Needs and support system identification
• Customized and adjustable care plans, depending on participant needs
• Purchasing of services to provide necessary supports. Cost sharing may be required for some individuals
• Follow-up and monitoring of participant situation
