Disease Management Program

Each patient receives an individualized care and education plan that is tailored to meet the individual needs of the patient and their family. The care and education plan along with concise progress updates are communicated to the physician following each contact.
In addition to patient specific reports we provide clinical outcome date for each disease condition being managed for your practice. These reports can be used to validate your practice patterns and to help you maximize your potential reimbursement. This reporting in conjunction with your HEDIS data can assist your practice in providing a continued level of optimal care.
