Care Resources
Care Resources is a Program of All-inclusive Care for the Elderly (PACE), a long-term comprehensive health care program that enables frail elderly adults to live at home in their communities for as long as medically and socially possible. Care Resources is committed to helping frail elderly adults maintain their independence, dignity and quality of life. Care Resources is a Health Plan which provides all medical care (24-hours a day), a Day Health Center, home care services and transportation.

Holland Home
Holland Home has been serving seniors since 1892. Located in Grand Rapids, Michigan, Holland Home includes three campuses providing the full continuum of care — independent and assisted living, skilled nursing and respite and memory care, as well as home health and hospice services.
