Community Outreach Representative

Responsibilities

Promotes all programs and services of Care Resources to increase referral volume. Responsible for strategically building effective relationships with referral sources; identifying and acting on opportunities to build referrals, and acting as a liaison for flow of information and problem resolution.

Skills required

Bachelor's degree in Marketing or health care field or licensed practical nurse (LPN) or Social Worker. Experience in promotions and sales as normally acquired through 3-5 yrs of work experience. Experience in fund development and grant writing. Must have a minimum of 1 yr of experience working with frail or elderly population. Self motivated and organized.
