RN Case Manager

Responsibilities

Responsible for performing in-person client assessments, which will include planning, implementing, coordinating, monitoring and evaluation of care plans to facilitate appropriate services across the continuum of care.

Skills required

Licensed RN in the state of Michigan. Minimum of 5 yrs of experience in a related field. Home care or case management experience is required.
